Stage 2: Unresponsive vigilance or vegetative state. Eye opening returns and a sleep-wake cycle commences. There is no sign of responsiveness. Approximately 2% of patients plateau at this stage. Stage 3: Mute responsiveness. The patient shows signs of responsiveness and may begin to follow commands. Alternatively, some patients’ verbal abilities may return before commands are followed.
